Requesting A Sponsored Account

This page may be used to submit a request to your sponsor for an official affiliation with CWRU.

Affiliate accounts expire after no more than one year and must be renewed annually.

Who can use this option?

The person that will be assigned the new affiliation needs to fill in the form so we can get the most accurate data possible.
To use this form you will need to have a CWRU sponsor that will approve the new affiliation.

When filling out this form you will need the following information:

• The user ID of your CWRU sponsor.
• If you have ever had a user ID from CWRU you will need to know what it is.
• And, of course, you will need to fill in some personal information about yourself.
