Requesting A Sponsored Account

This page may be used to submit a request to your sponsor for an official affiliation with CWRU.

Who can use this option?

To use this form you will need to have a CWRU sponsor that will approve the new affiliation.

When filling out this form you will need the following information:

The user ID of your CWRU sponsor.

If you have ever had a user ID from CWRU you will need to know what it is.

And, of course, you will need to fill in some personal information about yourself.
